How does a 6×6 risk matrix work?

Likelihood (1-6) × severity (1-6) gives a score from 1 to 36. The score falls into a band - low, moderate, high, very high or extreme - and each band has an action. The grid on the page shows every combination.

What are the band boundaries?

Up to 4 is low, 5-15 moderate, 16-24 very high once past high, and 25 and above extreme, as coded in the tool. They are a common convention, not a standard; if your own risk assessment procedure uses different bands, use those.

Is a score a risk assessment?

No. A score is one line of one. Identifying the hazard, who is harmed, the controls and the residual risk is the assessment, and a competent person does it. Pascoe Systems makes no compliance claim.
